Median sternotomy is the Gold Standard access for surgical revascularization of the myocardium, but it is not free of complications, especially in patients with multiple comorbidities. Minimally invasive access offers the advantage of avoiding sternotomy, achieving a more accelerated postoperative recovery, with less hospital stay time and a higher level of satisfaction with quality of life. We present the case of a 49-year-old male patient, diabetic, hypertensive, smoker, with multiarterial coronary artery disease, very symptomatic, who underwent surgical revascularization by left mini-thoracotomy.

La esternotomía media es el acceso “Gold Standard” para la revascularización quirúrgica del miocardio, pero no está exenta de complicaciones, sobre todo en pacientes con comorbilidades múltiples. El acceso mínimamente invasivo ofrece la ventaja de evitar la esternotomía, además de una recuperación posoperatoria más acelerada, con menos tiempo de estancia hospitalaria y un mayor nivel de satisfacción en la calidad de vida. Se presenta el caso de un paciente varón de 49 años, diabético, hipertenso, tabaquista, con enfermedad coronaria multiarterial, muy sintomático, quien fue revascularizado quirúrgicamente por minitoracotomía izquierda.

Evolución y complicaciones en pacientes postoperados de cirugía cardiaca con hipertensión arterial pulmonar en la unidad de cuidados intensivos cardiovascular en el Instituto Nacional Cardiovascular del año 2019. Un punto para recalcar que se ha visto en varios estudios, pacientes con HP persistente o residual es un factor de mayor morbilidad y mortalidad en el postoperatorio a los 2 años aproximadamente de seguimiento.

Objetivo: determinar la frecuencia del rechazo de atención por triaje y sus factores relacionados en el servicio de emergencia de un hospital de tercer nivel. Material y métodos: estudio transversal con una muestra consecutiva de consultantes evaluados por el servicio de triaje que no ingresaron a emergencia del Hospital Regional Lambayeque durante el período Febrero-Abril 2015. Resultados: la frecuencia de rechazo por triaje fue 64,68% (628/971). De estos, el 64,29% fueron mujeres, tuvieron un promedio de edad de 38,55 años (IC95 37,22-39,66), siendo la mayoría padres de los pacientes (37,26%). Los síntomas más frecuentes fueron fiebre (12,55%) y dolor abdominal (12,55%). Un 44,02% no consideró su problema una cuestión de vida o muerte. To assess mortality, major valve-related events (MAVRE), and other complications in the perioperative period and follow up in patients with aortic valve replacement (AVR) through minithoracotomy (MT). Methods. We retrospectively analyzed patients aged <80 who underwent AVR through MT between January 2017 and December 2021 in a national reference center in Lima, Peru. Patients undergoing other surgical approaches (mini-sternotomy, etc.), other concomitant cardiac procedures, redo, and emergency surgeries were excluded. We measured the variables (MAVRE, mortality, and other clinical variables) at 30 days and a mean follow-up of 12 months. Results.
