 The quality of the root canal filling is today considered a factor of great importance in the success of endodontic treatment. The three-dimensional filling with thermoplastic injection prevents percolation and microfiltration towards the peri-apical tissues of the root canal. With the help of a combined technique of vertical condensation and thermoplasticized gutta-percha, working times are reduced and we obtain favorable results, but it depends on an excellent biomechanical preparation that facilitates the entry of instruments at the time of obturation.The present clinical case focuses on performing an endodontic treatment with manual protaper biomechanical preparation, thermoplastic filling and vertical condensation in the dental clinic of the Equinoctial Technological University, in the city of Quito-Ecuador.  Cysts are pathological cavities with liquid, semi-liquid, solid or gaseous content, lined with epithelium on the inside and by connective tissue on the outside. The peri-apical cyst is a common inflammatory lesion of the jaws, which upon reaching a significant size, retention or even the displacement of adjacent teeth, is derived from the remains of Malassez. A case was presented of a patient who came to the dental office, asymptomatic with the presence of leaking restorations. The clinical examination revealed soft palatal tissue without bone support at the level of pieces # 11 and # 12; radiographically, a hypodense, well-defined radiolucent image was evidenced, including vestibular and palatal tables, total palatal bone lysis and mucosal involvement.
