El HTLV-1 es un retrovirus endémico en el Perú, ocasionalmente asociado con algunas infecciones oportunistas aisladas. Presentamos el caso de un avicultor de 41 años con antecedentes de tuberculosis pulmonar tratado hace 6 meses. Ingresó en emergencia por alteración de la conciencia, disartria y diarrea acuosa. El examen reveló placas confluentes en la cavidad oral, lesiones maculopapulares violáceas y placas costrosas. La biopsia de piel confirmó el sarcoma de Kaposi y la sarna costrosa; Además, las linfadenopatías cervicales mostraron evidencia de BAAR (++) en el estudio microscópico con prueba de auramina, y en el examen de las heces con tinción de Zielh Nielsen modificada, se observó oistato de Cystoisospora belli y tratamiento antituberculoso con trimetropin / sulfametozaxol.
