El Derecho a la Salud debe ser entendido no sólo como la ausencia de enfermedades sino como el disfrute del grado máximo de bienestar físico, social y espiritual. Pero para que este derecho so sea solamente una afirmación retórica sino una hermosa realidad en nuestro país, será necesario llevar a cabo una serie de acciones y de disposiciones armónicas y coordinadas de carácter intersectorial, en los campos de la economía del trabajo, de la alimentación, de la educaci´ón, del saneamiento ambiental y otros, además de las que corresponden al campo estrictamente propio del sector salud.

This is a case of 65 year old male with past medical history significant for coronary artery disease and severe mitral valve prolapse for which he underwent simultaneous coronary artery bypass graft using the saphenous vein, mitral valve replacement with warfarin the patient wa discharged home. Three months postoperatively the patient developed apparent “hematuria”. Physical examination and extensive laboratory analysis were unremarkable in determinig an etiology. After referred, further studies ilustrared a diagnosis of hemoglobinuria. Transthoracic echocardiogram revealed a perivalvular leakage with a normally functioning St. Jude mitral valve replacement. Clinical course was uneventful and the hemoglobinuria resolved spontaneously. The effect of 9-alpha-fluorohy-drocortisone, a mineralocorticoid with minimal glucocorticoide activity, was studied in 40 patients with chronic or orthostatic arterial hypotension. The follow-up varied from 1 to 24 months. Periodic clinical ecaluation included blood pressure and puise measurements on lying, sitting and standing positions, a Schellong's postural test and weight variations. Serial determinations of serum electrolytes and urinary excretion of 17-ketosteroids and 17-hydroxycorticosteroids were also performed. Marked improvement was observed in 25 patients (62.5%) and moderate improvement in 14 (35%). The Schellong's postural test was abnormal in 37 patients (92%) prior to therapy and became normal in 34 (85%) after treatment.The average initial dose of the drug was 0.2 mg. daily.
