Los datos, obtenidos de las historias clínicas del periodo 2023-2024, se registraron en fichas de recolección diseñadas para el estudio y se analizaron con IBM SPSS Statistics 29. Resultados: La adherencia al tratamiento antihipertensivo se asocia significativamente con la presencia de complicaciones. El 84,4% de los pacientes con complicaciones presentó mala adherencia, frente al 45,6% en el grupo sin complicaciones (X2= 18,691, p<0,001). Además, el riesgo de desarrollar complicaciones fue 6,49 veces mayor en pacientes con mala adherencia (OR=6,49; IC 95%: 2,62 - 16,06). La edad, el tiempo desde el diagnóstico (p<0,001) y el sexo masculino (64,4% p=0,007) también influyeron en la prevalencia de complicaciones, mientras que el grado de instrucción no mostró relevancia estadística. Conclusión: La mala adherencia al tratamiento antihipertensivo incrementa más de seis veces el riesgo de desarrollar complicaciones asociadas a la hipertensión arterial

Materials and Methods: An analytical, descriptive, case-control study was conducted with 135 hypertensive patients from the arterial hypertension program at the Víctor Larco Herrera Polyclinic – ESSALUD, including 45 cases (with complications) and 90 controls (without complications). Data, obtained from medical records covering the 2023-2024 period, were recorded in data collection forms designed for the study and analyzed using IBM SPSS Statistics 29. Results: Adherence to antihypertensive treatment was significantly associated with the presence of complications. Among patients with complications, 84.4% had poor adherence, compared to 45.6% in the group without complications (X² = 18.691, p < 0.001). Additionally, the risk of developing complications was 6.49 times higher in patients with poor adherence (OR = 6.49; CI 95%: 2.62 - 16.06). Age, time since diagnosis (p < 0.001), and male sex (64.4%, p = 0.007) also influenced the prevalence of complications, whereas the level of education showed no statistical significance.
