Objetivo: Determinar los factores maternos asociados a histerectomía de emergencia en puérperas atendidas en el Hospital III-1 José Cayetano Heredia, Piura, durante el período 2021-2024. Materiales y métodos: Se realizó un estudio observacional analítico de casos y controles en el Hospital III-1 José Cayetano Heredia (EsSalud, Piura), entre 2021 y 2024. Se incluyeron pacientes con y sin histerectomía periparto de emergencia. La información se recolectó desde historias clínicas mediante una ficha estructurada y se analizó con STATA v.21 y R, empleando regresión logística multivariada. Resultados: Se incluyeron 90 pacientes. Los factores asociados significativamente a histerectomía fueron comorbilidades maternas, atonía uterina y acretismo placentario. El forest plot evidenció mayor riesgo con OR ajustados mayores a 4 para estos factores. Conclusión: La histerectomía periparto de emergencia se asoció principalmente a condiciones obstétricas prevenibles, lo que resalta la importancia del diagnóstico prenatal y la atención oportuna en gestantes de alto riesgo

Its incidence varies according to maternal factors and perinatal conditions. Objective: To determine maternal factors associated with emergency hysterectomy in postpartum women treated at Hospital III-1 José Cayetano Heredia, Piura, during the period 2021–2024. Materials and methods: An observational analytical case-control study was conducted at Hospital III-1 José Cayetano Heredia (EsSalud, Piura) between 2021 and 2024. Patients with and without emergency peripartum hysterectomy were included. Data were collected from medical records using a structured form and analyzed with STATA v.21 and R, using multivariate logistic regression. Results: A total of 90 patients were included. Maternal comorbidities, uterine atony, and placental accreta were significantly associated with hysterectomy. The forest plot showed a higher risk with adjusted ORs greater than 4 for these factors.
