Determinar la seguridad entre la colecistectomía laparoscópica subtotal y la colecistectomía laparoscópica total en el manejo de la colecistectomía difícil en pacientes con colecistitis litiásica aguda Métodos: Se llevó a cabo un estudio observacional de cohortes retrospectivas poblacionales en el Hospital de Alta Complejidad ““Virgen de la Puerta““ durante el periodo de enero de 2015 a octubre de 2023, en pacientes adultos operados y considerados colecistectomía laparoscópica difícil. Se consideraron variables como edad, sexo, presencia de diabetes mellitus tipo 2, hipertensión arterial, score ASA, tiempo de enfermedad, tiempo operatorio, conversión a cirugía abierta, estancia hospitalaria y Escala de Parkland (categoría 3 o 4). Se aplicaron criterios de inclusión y exclusión. Resultados: Se conformaron dos grupos de estudio: uno sometido a colecistectomía laparoscópica subtotal y otro a colecistectomía laparoscópica total. En el grupo de colecistectomía subtotal, hubo una mayor proporción de pacientes de sexo masculino y una edad promedio ligeramente mayor en comparación con el grupo de colecistectomía total. El tiempo operatorio promedio fue significativamente mayor en el grupo de colecistectomía laparoscópica subtotal. En cuanto a las complicaciones, se observaron diferencias en las tasas de complicaciones intraoperatorias y postoperatorias entre ambos grupos. Conclusiones: Los resultados sugieren que la colecistectomía laparoscópica subtotal y la colecistectomía laparoscópica total tienen diferencias significativas en términos de duración de la operación; no se encontraron diferencias en el resto de variables. Se necesitan más estudios para determinar la seguridad de ambas técnicas en el manejo de la colecistectomía difícil en pacientes con colecistitis litiásica aguda |

Methods: An observational study of population-based retrospective cohorts was conducted at the ““Virgen de la Puerta““ High Complexity Hospital from January 2015 to October 2023. Adult patients who underwent difficult laparoscopic cholecystectomy were included in the study. Variables such as age, gender, presence of type 2 diabetes mellitus, hypertension, ASA score, duration of illness, operative time, conversion to open surgery, hospital stay were considered and Parkland Scale (category 3 or 4). Inclusion and exclusion criteria were applied. Results: Two study groups were formed: one underwent subtotal laparoscopic cholecystectomy, and the other underwent total laparoscopic cholecystectomy. In the subtotal cholecystectomy group, there was a higher proportion of male patients and a slightly older average age compared to the total cholecystectomy group. The average operative time was significantly longer in the subtotal laparoscopic cholecystectomy group. Regarding complications, differences in the rates of intraoperative and postoperative complications were observed between both groups. Conclusions: The results suggest that subtotal laparoscopic cholecystectomy and total laparoscopic cholecystectomy have significant differences in terms of operative time; no differences were found in the rest of the variables.
