The aim of the present study was to evaluate the pregnancy and implantation rates when embryo transfer was carried out in cleavage-stage embryos in day 3 or in blastocyst-stage embryos in day 5 in an assisted reproduction program with oocyte donation. 1667 cycles of assisted reproduction were studied retrospectively, 594 cycles with embryo transfer of day 3 and 1073 cycles of day 5. Implantation (29,09 % vs 44,85 %), pregnancy (51,01 % vs 61,7%) and delivery (44,28 % vs 52,19 %) rates were significantly higher with the blastocyst group transfer compared to cleavage-stage embryos group. Instead, the higher-order multiple pregnancy rates were significantly lower with the blastocyst group transfer. In conclusion, the results of this study support the blastocyst transfer in fEn las técnicas de reproducción asistida, la transferencia embrionaria se ha llevado a cabo en estadio de clivaje al segundo o tercer día después de la aspiración de ovocitos. Sin embargo, en los últimos años se ha introducido la transferencia embrionaria en estadio de blastocisto. El objetivo del presente estudio fue evaluar las tasas de embarazo e implantación cuando se transfieren embriones en el día 3 en estadio de clivaje y en el día 5 en estadio de blastocisto, dentro de un programa de reproducción asistida con donación de ovocitos. Se estudiaron de manera retrospectiva un total de 1667 ciclos; de los cuales 594 ciclos fueron con transferencia embrionaria en día 3, y 1073 ciclos, en día 5. Las tasas de implantación (29,09% vs 44,85%), embarazo (51,01% vs 61,7%) y parto (44,28% vs 52,19%) fueron significativamente mayores con la transferencia de embriones en estadio de blastocisto comparada a embriones en estadio de clivaje.
