It aims to provide plastic surgeons with confidence and flexibility in covering large thoracic soft tissue defects, thereby achieving safe and aesthetically acceptable results. Recommendations and lessons on its use are provided, demonstrating its adaptability to various defect types, ranging from oncological to traumatic lesions, and reducing postoperative mortality while promoting faster recovery.El colgajo en isla de Keystone es una técnica quirúrgica reconstructiva empleada para cerrar defectos importantes en la cabeza, extremidades y tronco. Esta técnica permite el uso de piel adyacente con una excelente correspondencia de color natural, requiere menos tiempo operatorio y evita procedimientos más complejos, como colgajos libres o injertos. Este artículo tuvo como objetivo describir la experiencia con el uso del colgajo Keystone en la reconstrucción de defectos en la pared torácica, destacando su versatilidad gracias a las múltiples perforantes musculocutáneas que ofrece. Se busca proporcionar confianza y libertad al cirujano plástico para la cobertura de grandes defectos de tejidos blandos en el tórax, obteniendo resultados seguros y estéticamente aceptables.
