We present the case of an 18-year-old female patient, with no relevant medical history, who ingested 10 tablets of 500 mg ciprofloxacin in a single intake. She subsequently developed pelvic abdominal pain, nephritic syndrome, and acute kidney injury classified as AKIN stage 3. Urinalysis revealed >100 red blood cells per field and 1+ proteinuria; blood tests showed urea 157.7 mg/dL and creatinine 10.04 mg/dL. Kidney biopsy revealed tubulointerstitial nephritis with toxic tubular damage, a lymphomononuclear infiltrate with eosinophils, and mild fibrosis. She was treated with pulses of methylprednisolone, oral prednisone (0.5 mg/kg/day), and ranitidine, achieving full recovery: creatinine 1.1 mg/dL and resolution of microhematuria. This case highlights the importance of considering this rare complication in patients with acute kidney injury following fluoroquinolone use.El ciprofloxacino es un antibiótico de uso común que puede ocasionar efectos adversos poco frecuentes como la nefritis tubulointersticial aguda, posiblemente asociada a una reacción inmunológica que desencadena inflamación intersticial y daño tubular. Presentamos el caso de una mujer de 18 años, sin antecedentes patológicos, que ingirió de forma masiva 10 tabletas de ciprofloxacino de 500 mg. Posteriormente, desarrolló dolor abdominal pélvico, síndrome nefrítico e injuria renal aguda estadio AKIN 3. El examen de orina mostró >100 hematíes por campo y proteinuria 1+; en sangre la urea fue 157,7 mg/dL y la creatinina 10,04 mg/dL. La biopsia reveló nefritis tubulointersticial con daño tubular tóxico, infiltrado linfomononuclear con eosinófilos y fibrosis leve. Fue tratada con pulsos de metilprednisolona, prednisona oral (0,5 mg/kg/día) y ranitidina, con recuperación completa: creatinina 1,1 mg/dL y resolución de la microhematuria.
