Objective. To analyze the burden of CKD in the Peruvian population, focusing on productivity loss and its economic impact. Methods. Estimates from the 2021 Global Burden of Disease (GBD) Study for Peru from 1990 to 2021 were used, disaggregated by age and sex. Indicators included mortality, Years of Life Lost (YLL), Years Lived with Disability (YLD), and Disability-Adjusted Life Years (DALYs). Absolute numbers and age-standardized rates with 95% confidence intervals were calculated. Economic data from the World Bank were used to estimate the economic burden of productivity loss (EBPL) using the human capital approach. Results were expressed in constant U.S. dollars, Peruvian soles, and as a percentage of GDP. Results. Between 1990 and 2021, mortality, YLL, DALYs, and EBPL due to CKD increased, while YLD decreased. The highest burden was observed in individuals over 80 years old. EBPL increased more than fivefold, both in absolute terms and as a share of GDP. Conclusions: The burden of CKD has increased in Peru, especially among older adults. YLL accounted for most of the DALYs. The gap between the rising economic impact and its weight on GDP highlights the urgent need to strengthen specialized care and health system financing.

La enfermedad renal crónica (ERC) afecta al 11–15 % de los adultos a nivel mundial, y su impacto sigue creciendo. En Perú, la detección tardía y las limitaciones en el acceso al tratamiento agravan su carga. Objetivo. Analizar la carga de la ERC en la población peruana, enfocándose en la pérdida de productividad y su impacto económico.  Métodos. Se utilizaron estimaciones del Estudio de Carga Global de Enfermedades (GBD) 2021 para Perú entre 1990 y 2021, desagregadas por edad y género. Los indicadores incluyeron mortalidad, AVP, AVD y AVAD. Se calcularon cifras absolutas y tasas ajustadas por edad con su intervalo de confianza al 95 %. Con datos del Banco Mundial, se estimó la carga económica por pérdida de productividad (CEPP) usando el enfoque de capital humano. Los resultados se expresaron en dólares constantes, soles constantes y porcentaje del PBI. Resultados. Entre 1990 y 2021, aumentaron la mortalidad, AVP, AVAD y CEPP por ERC, mientras los AVD disminuyeron. La mayor carga se observó en personas mayores de 80 años. La CEPP se multiplicó por más de cinco en cifras absolutas y como proporción del PBI. Conclusiones. La carga de la ERC ha aumentado en Perú, especialmente en adultos mayores. Predominaron los AVP dentro de los AVAD.
