description Introduction. Consultations outside the Maternal Intensive Care Unit (ICU) enable early identification of critical complicationsin obstetric patients, preventing clinical deterioration. Objective. To identify the characteristics of non-ICU consultationsand their relationship with maternal Near Miss events and ICU admission. Methods. An observational, correlational,and retrospective study was conducted, including 768 patients assessed outside the ICU at the Instituto Nacional MaternoPerinatal (INMP) between January and December 2022. Sociodemographic and clinical variables were collected. Bivariateand multivariate analyses were performed to evaluate the association between organ dysfunctions and the outcomesof Near Miss events and ICU admission. Adjusted risk ratios (RR) with 95% confidence intervals (CI95%) were estimated.A p-value <0.05 was considered statistically significant. Results. A total of 54% of the patients experienced Near Missevents. The main causes were hypertensive disorders (46%) and hemorrhages (23%). Respiratory dysfunctions (adjustedOR: 6.36; CI95%: 3.96–10.23) and electrolyte imbalances (adjusted OR: 3.17; CI95%: 2.07–4.87) were significantly associatedwith ICU admission. Direct request for transfer to ICU showed the strongest association with this outcome (adjustedRR: 2.16; CI95%: 1.80–2.60; p<0.001). Conclusion. Extramural consultations conducted by the ICU play a crucial role inidentifying obstetric complications and detecting Near Miss events. Strengthening ICU referral criteria and early responseis key to optimizing care and reducing morbidity and mortality in critical obstetric care.

Las consultas fuera de la Unidad de Cuidados Intensivos (UCI) materna son importantes para la identificacióntemprana de complicaciones críticas en pacientes obstétricas, previniendo su deterioro. Objetivo. Identificar lascaracterísticas de las atenciones fuera de UCI y su relación con los eventos Near Miss maternos y los ingresos a UCI.Métodos. Se realizó un estudio observacional, correlacional y retrospectivo con 768 pacientes atendidas fuera de la UCIen el Instituto Nacional Materno Perinatal entre enero y diciembre de 2022. Se recopilaron variables sociodemográficasy clínicas. Se utilizaron análisis bivariados y multivariados para evaluar la asociación entre disfunciones orgánicas y losdesenlaces Near Miss e ingreso a UCI. Se estimaron razones de riesgo (RR) ajustadas con intervalos de confianza al95 % (IC95%). Se consideró significativo un valor de p<0,05. Resultados. El 54 % de las interconsultas se trataron deeventos Near Miss. Los principales patologías que motivaron la consulta fueron los trastornos hipertensivos (53,9%). Enel analisis multivariado las patologías con más riesgo de ser casos de NEAR MISS fueron la sepsis con RR ajustado de1,77 (1,47 - 2,13), seguido de las hemorragias obstétricas 1,51 (1,33 - 1,71). En cuanto a la solicitud de pase de ingresoa UCI tuvo un RR ajustado: 1,03 (0,94 - 1,13) p<0,493 no significativo. Por otro lado, las disfunciones respiratorias (RRajustado: 1,5 (1,30 - 1,86) y los trastornos electrolíticos (RR ajustado: 1,92; (1,60 - 2,29) se asociaron con el ingreso aUCI. La solicitud de pase directo a UCI tuvo la asociación significativa más fuerte con este último desenlace (RR ajustado:2,16 (1,80–2,60) p<0,001). Conclusión. Las consultas extramurales de UCI desempeñan un papel crucial en la identificaciónde complicaciones obstétricas y detección de eventos Near Miss. EL motivo de solicitud de interconsulta a UCIno necesariamente significa la identificación de un caso Near Miss.
