We present thecase of a 44-year-old male patient, a gardener by occupation, who was bitten on the left thirdfinger by a snake in Cieneguilla. Initially, no antivenom was available at the nearest health center;therefore, he was transferred to Hospital Edgardo Rebagliati Martins on the same day. Despitereceiving immediate medical attention and antivenom treatment upon arrival at the hospital, hedeveloped common local symptoms such as pain, edema, and ecchymosis, but did not exhibit anysevere long-term complications. Although the patient did not present with serious complications,the variability in the clinical presentation of snakebites can complicate their management,highlighting the importance of individualized evaluation and treatment. The patient experienceda favorable recovery and was discharged after six days, with no complications. This case pointsout the importance of prompt medical attention and access to antivenom, especially in areaswhere snakebites are endemic. In addition, it underscores the need for increased communityeducation on local species and bite prevention, particularly for agricultural workers who facea higher risk. The definitive diagnosis of ophidism caused by Bothrops pictus, recorded in themedical history, was made based on the characteristic clinical presentation, laboratory findings, the snake’s features described by the patient, and the geographic-epidemiological characteristics. |

Presentamos el caso de un paciente varón de 44 años, jardinero de ocupación, quien sufrió una mordedura en el tercer dedo izquierdo por una serpiente, en Cieneguilla. Inicialmente, no había suero antiofídico disponible en el centro de salud más cercano, por lo que fue trasladado al Hospital Edgardo Rebagliati Martins el mismo día. A pesar de recibir atención médica inmediata y tratamiento antiofídico tras su llegada al hospital, desarrolló síntomas locales comunes como dolor, edema y equimosis, pero no manifestó complicaciones graves a largo plazo. Si bien el paciente no presentó complicaciones graves, la variabilidad en la presentación clínica de las mordeduras de serpientes puede complicar su manejo, lo que resalta la importancia de unaevaluación y tratamiento individualizados. El paciente experimentó una recuperación favorable y fue dado de alta después de seis días, sin complicaciones. El presente caso destaca la importancia de una rápida atención médica y el acceso al suero antiofídico, especialmente en áreas donde las mordeduras de serpientes son endémicas. Además, subraya la necesidad de una mayor educación comunitaria sobre las especies locales y la prevención de mordeduras, especialmente para trabajadores agrícolas que enfrentan un mayor riesgo. El diagnóstico definitivo de ofidismo por Bothrops pictus registrado en la historia clínica se obtuvo por la presentación clínica característica, hallazgos de laboratorio, características de la serpiente descritas por el paciente y características geográfico-epidemiológicas.Universidad de San Martín de Porres.
