description Objective: To report the results of pulse oximetry screening for congenital heart diseases in healthy newborns during the transitional period between birth and discharge. Those with positive screening results were referred for echocardiographic evaluation aimed at a specific diagnosis. Materials and methods: A retrospective, descriptive, observational and cross-sectional study. Data were obtained from screening 4,897 newborns who were admitted to the nursery of the Obstetrics and Gynecology Unit at Hospital Universitario de Guayaquil, within the first 24 hours of birth, all diagnosed as healthy newborns. As part of the protocol, cardiac screening was performed on all newborns at admission and discharge. Newborns with saturation levels below 95 % on pulse oximetry underwent consecutive measurements at one-hour intervals, with a total of three readings. Additionally, those with more than a 3 % difference in saturation between preductal and postductal readings were also monitored. Newborns with positive screening results underwent transthoracic echocardiography to evaluate for heart defects. Results: Among the 4,897 newborns, 626 (12.8 %) had a positive neonatal cardiac screening, out of which 497 (79.4 %) were diagnosed with some form of structural heart defect. Persistent ductus arteriosus was the most frequent f inding, with 127 cases (25.55 %). Critical malformations such as aortic aneurysms and coarctation of the aorta were reported in 0.8 % and 1.6 %, respectively. Conclusions: Screening for congenital heart diseases using pulse oximetry during the transitional period prior to discharge is essential for the early detection of congenital heart diseases in healthy newborns.

Aquellos que presentaron tamizaje positivo fueron referidos a una evaluación ecocardiográfica para el diagnóstico específico. Materiales y métodos: El presente estudio es de carácter retrospectivo y de tipo descriptivo, observacional de corte transversal. Se obtuvo datos de 4897 recién nacidos que pasaron a la sala de alojamiento conjunto en la Unidad de Ginecología y Obstetricia del hospital Universitario, dentro de las primeras 24 horas de nacimiento, con diagnóstico de neonatos sanos. Al ingreso al alojamiento conjunto, se sometió al tamizaje cardiológico a los recién nacidos como parte del protocolo, tanto al ingreso como para el alta hospitalaria. A los que saturaron menos del 95 % en la oximetría de pulso, se les sometió a mediciones sucesivas en intervalos de una hora por tres ocasiones, igualmente a aquellos con una diferencia de saturación superior al 3 % entre la saturación preductal y la posductal. Los neonatos con tamizaje positivo se sometieron a ecocardiografía transtorácica para evaluar los defectos cardiacos. Resultados: De 4897 recién nacidos se encontraron 626 (12,8 %) con tamizaje cardiológico neonatal positivo; de ellos 497 (79,4 %) reportaron un ecocardiograma con algún defecto estructural. La persistencia del conducto arterioso fue el hallazgo más frecuente en 127 casos (25,55 %). Se encontraron casos de aneurismas de aorta y coartación aórtica como malformaciones críticas (0,8 % y 1,6 % respectivamente). Conclusiones: El tamizaje de cardiopatías congénitas mediante pulsioximetría en el periodo de transición previo a su alta es importante para el diagnóstico precoz de cardiopatías congénitas en los recién nacidos considerados sanos.Universidad de San Martín de Porres.
