We present the case of a 41-year-old woman who presented with abdominal pain. Ultrasound showed cholelithiasis and choledocholithiasis; and the cholangioresonance study showed bile duct dilation and distal stenosis. A retrograde endoscopic cholangiopancreatography was made and we removed two parasites similar to hepatic flukes. Subsequently, the cholecystectomy could not be performed due to delay and request for voluntary discharge. In our environment, even today, there is a need to suspect fascioliasis, in the case of a compatible clinical of choledocholithiasis in patients from endemic regions. Therefore, knowing the best study alternatives and appropriate treatment according to the phase of the disease is important to effectively combat this disease.El Perú es uno de los países con mayor distribución regional de fascioliasis humana en el mundo, provocada por F. hepática. Fue considerada una enfermedad emergente y puede tener alta morbilidad. Presentamos el caso de una mujer de 41 años que acudió con cuadro de dolor abdominal. La ecografía mostró colelitiasis y coledocolitiasis; y el estudio de colangioresonancia evidenció dilatación de vías biliares y estenosis distal. Se realizó una colangiopancreatografía endoscopia retrógrada donde se extrajeron dos parásitos semejantes a fasciolas hepáticas. Posteriormente no se logró realizar la colecistectomía por demora y solicitud de alta voluntaria. En nuestro medio, aún en la actualidad, existe la necesidad de sospechar la fascioliasis, frente a un cuadro clínico compatible de coledocolitiasis en pacientes procedentes de regiones endémicas.
