Four cases were taken part in the study period; with same quantity of cases for males and females. With ages between 2 and 17 years, and an average of 11.2 years. The four cases were from the inner states of Peru (Piura, Tumbes, Cajamarca and Junín). We present mandible resections as long as 15 cm for one side. All the cases underwent a surgical procedure with nasotracheal intubation; different mandible resections and a free fibula flap reconstruction, intermaxillary fixation and osteosynthesis with a mandible titanium plate and screws. We report an average blood loss of 467.5 cc, with a rank between 220 and 800 cc. Our transfusion average was 2.25 red blood cells pack by patient.Introducción: la reconstrucción mandibular enmarcada en la cirugía de cabeza y cuello, constituye un tema de constante innovación alimentado por el intenso debate e innovación científica que permiten el tratamiento de procesos morbido tumorales muchas veces no exentos de secuelas estéticas y funcionales.Casos clínicos: se revisaron las intervenciones quirúrgicas de patologías tumorales mandibulares que requirieron una reconstrucción microquirúrgica con colgajo libre de peroné entre enero 2004 hasta agosto 2008; en el servicio de Cirugía de Cabeza, Cuello y Maxilofacial del Instituto Especializado de Salud del Niño. Se intervinieron 04 pacientes durante el periodo en estudio, con igual cantidad de casos para ambos sexos. Las edades van desde 2 hasta los 17 años con un promedio de 11.2 años. Siendo los lugares de procedencia Piura, Tumbes, Cajamarca y Junín. Presentamos tumoraciones que requirieron resecciones amplias siendo necesario, la reconstrucción de defectos hasta de 15 cm de una hemimandíbula. Todos los pacientes fueron sometidos a tratamiento quirúrgico con intubación nasotraqueal; realizándose resecciones de diferentes segmentos mandibulares y reconstrucción con colgajo libre de peroné más osteosíntesis con placa de reconstrucción y tornillos de titanio, previa fijación intermaxilar. La pérdida sanguínea en promedio fue de 467.5 cc, en un rango de entre 220 y 800 cc.
