Data from NOTICOVID and SINADEF were utilized. Crude (HRc) and adjusted hazard ratios (HRa) were calculated to assess the association between immunization status and COVID-19 mortality. Vaccine effectiveness (VE) was computed as VE% = (1 – HR)*100. A total of 972 adults (52.9% female) with a median age of 42 years were analyzed; 9.4% had comorbidities. Booster immunization demonstrated a significantly lower hazard of death compared to unvaccinated individuals (HRa: 0.06; 95% CI: 0.02–0.23). Booster vaccination is concluded as a critical strategy to reduce COVID-19 mortality and to strengthen public immunization policies.El objetivo del estudio fue estimar la efectividad vacunal contra el SARS-CoV-2 para reducir la letalidad por COVID-19 en adultos con inmunización parcial, completa y de refuerzo frente a los no inmunizados. Se realizó un estudio de cohorte retrospectiva que incluyó a adultos atendidos por la red asistencial Ancash–EsSalud (Perú), entre enero y marzo de 2022, con infección confirmada por RT-PCR y/o antígeno. Se emplearon datos de NOTICOVID y SINADEF. Se estimaron Hazard ratios crudos(HRc) y ajustados(HRa) para la asociación entre inmunización y letalidad por COVID-19. La efectividad vacunal (EV) se calculó mediante EV% = (1 – HR)*100. Se analizaron 972 adultos (52,9% mujeres), con una mediana de edad de 42 años; el 9,4% presentó comorbilidad. La inmunización de refuerzo mostró un menor Hazard de muerte frente a los no inmunizados (HRa: 0,06; IC95%: 0,02–0,23).
