All of these aspects must be carried out within a, equity, universality, solidarity, accessibility, participation, quality, and efficiency framework .Dado que los trastornos neuropsiquiátricos constituyen la primera causa de carga de enfermedad en nuestro país y que ello se traduce en altas tasas de prevalencia en la población general; y frente a lo cual paradójicamente el Estado no ha asumido su responsabilidad; se plantea la necesidad de impulsar un proceso de reforma de la salud mental que tenga como ejes fundamentales: 1) La asunción por parte del MINSA de su rol rector de la salud mental, priorizándolo como política pública, contando para ello con un Órgano de Dirección de Salud Mental; 2) Reestructuración de la atención psiquiátrica ligándola a la Atención Primaria de Salud en los marcos de los sistemas locales de salud que permita la promoción de modelos alternativos centrados en la comunidad y dentro de sus redes sociales, 3) Salvaguarda de la dignidad personal y los derechos humanos de los usuarios; 4) Modernización de las actuales instituciones psiquiátricas del más alto nivel cambiando su rol hegemónico actual a otro de subespecialización, asesoría, docencia e investigación.
