Gerencia Departamental AyacuchoNuestro objetivo fue establecer si Fentanyl Intratecal ofrece una adecuada alternativa de costo beneficio frente a otras opciones clásicas y recientes de analgesia obstétrica con mínimo efecto secundario, en nuestro hospital con limitada cobertura de anestesia. Pacientes de bajo riesgo mayores de 35 semanas de gestación con 5 ó más centímetros de dilatación se les administró intratecal 25 mg. de Fentanyl más de 2.5 mg de bupivacaina. Por un periodo de cinco meses se hizo una revisión retroactiva a todos los pacientes . que recibieron analgesia intratecal, _ comparando con el subsiguiente paciente de bajo riesgo no recibió analgesia. Se colocó analgesia a 30 pacientes (15% del total de partos). En ambos grupos no hubo diferencias significativas, en cuanto al índice de cesáreas, APGAR al nacer. Tres pacientes (10%) requirieron vacum en el grupo de estudio, fue ligeramente superior que 1 (3%) de grupo control. El promedio del progreso de labor no fue afectado. Significativamente. más pacientes que usaron analgesia experimentaron prurito. No se presentó otros efectos secundarios. Veintinueve (97%) de los 30 pacientes reportaron excelente alivio de dolor, uno fue satisfactorio. En nuestro hospital con limitados servicios de anestesia, Fentanyl intratecal ofrece excelente alivio de dolor con manejables efectos secundarios y sin efectos adversos al alta.Our purpose was to stablish if Fentanyl Itratecal es a valid alternative versus other clasic options in obstetric analgesic drugs, with low secundary efects; in our hospital with limitade capacity in anesthesy. We use Fentanyl25 ug. plus bupivacaina intratecal in patients with low risk, with more than 35 weeks of pregnancy with 5 or more cms of dilatation. Then we compared results during 5 months versus patients with no analgesic treatment. We used analgesic treatment to 30 patients (15% from all bom). Results: No important differences in APGAR, and in number of cesarys. Three patients (10%) needad vacum. One (3%) in the control group. The progress promedy was not affected. Twenty-nine (97%) patients reported no pain.
