HRT in the perimenopause. An alternative for patients that do not wish to menstruate. Preliminary study

OBJETIVE: The present work aims at evaluating a way to administrate a drug named tibolone, among a group of patients having it as the unique therapy of hormonal replacement, but who still have some type of uterine bleeding and who are not still having definite menopause. The indication for the use of this drug is the presence of amenorrhea for at least one year. DESIGN: The idea is to administrate a combined hormonal treatment with pills derived from estrogens as well as progesterone in order to reduce to the minimal expression the endometrium, accompanied by an ultrasound assessment. RESULTS: It has been found that fifty percent of the studied patients responded satisfactorily to our therapeutic scheme. CONCLUSION: The fifty percent of the patients who have less than 7mm of endometrium, could use our therapy.
OBJETIVO: El presente trabajo está dirigido a evaluar una manera de administrar un fármaco, la tibolona, para aquel grupo de pacientes que tendrían únicamente esa opción como terapia hormonal de reemplazo, pero que aún presentan algún tipo desangrado uterino y aún no están definitivamente en la menopausia. Debido a que la indicación para el uso de este fármaco, es la amenorrea mínima de un año. DISEÑO: La idea es administrar un tratamiento hormonal combinado con píldoras de derivados del estrógeno y la progesterona y abatir el endometrio, con la debida evaluación ecográfica, llevándolo a una atrofia o disminución mínima y luego prescribir la tibolona. RESULTADOS: Hemos encontrado que el cincuenta por ciento de las pacientes estudiadas respondieron satisfactoriamente a nuestro esquema terapéutico. CONCLUSIONES: La mitad de las pacientes usando nuestro esquema terapéutico, con endometrio abatido a menos de 7 mm, podrían beneficiarse con nuestra propuesta.
