Se incluyeron 75 casos (pacientes con queratopatía bullosa pseudofáquica) y 150 controles (sin queratopatía bullosa pseudofáquica), seleccionados con una relación 1:2. Se aplicaron pruebas de U de Mann-Whitney, chi-cuadrado y regresión logística, considerando un valor de p < 0,05 como significativo. RESULTADOS: En el análisis multivariado, los factores independientes asociados significativamente al desarrollo de QBP fueron: una menor densidad celular endotelial preoperatoria (OR = 0,998; IC 95%: 0,996-0,999), la presencia de múltiples comorbilidades sistémicas (OR = 7,28; IC 95%: 3,01-17,62), el diagnóstico de glaucoma preexistente (OR = 7,17; IC 95%: 2,62-19,64), el antecedente de cirugía intraocular previa (OR = 5,08; IC 95%: 2,19-11,80) y la ocurrencia de complicaciones intraoperatorias (OR = 8,93; IC 95%: 1,51-52,83). No se encontró asociación significativa con la edad, el síndrome de pseudoexfoliación, la dureza nuclear (LOCS III), las variables biométricas o el tipo de cirujano. CONCLUSIONES: La QBP es una complicación multifactorial. Los hallazgos resaltan la importancia de una evaluación preoperatoria integral, que incluya la densidad endotelial y el historial de comorbilidades oculares y sistémicas, para identificar pacientes en riesgo y optimizar las estrategias quirúrgicas preventivas

MATERIAL AND METHOD: An observational, analytical, retrospective, case- control study. Seventy-five cases (patients with pseudophakic bullous keratopathy) and 150 controls (without pseudophakic bullous keratopathy) were included, selected with a 1:2 ratio. Mann-Whitney U test, chi-square test, and logistic regression tests were applied, with a p value < 0.05 considered significant. RESULTS: In the multivariate analysis, the independent factors significantly associated with the development of PBK were: lower preoperative endothelial cell density (OR = 0.998; 95% CI: 0.996-0.999), the presence of multiple systemic comorbidities (OR = 7.28; 95% CI: 3.01-17.62), a diagnosis of pre-existing glaucoma (OR = 7.17; 95% CI: 2.62-19.64), a history of previous intraocular surgery (OR = 5.08; 95% CI: 2.19-11.80), and the occurrence of intraoperative complications (OR = 8.93; 95% CI: 1.51-52.83). No significant association was found with age, pseudoexfoliation syndrome, nuclear hardness (LOCS III), biometric variables, or type of surgeon. CONCLUSIONS: PBK is a multifactorial complication.
