Summary:The objective of this report was to evaluate the clinical and microbiological effectiveness of the pharmacotherapy recorded in the medical records of the patients of the Edgardo Rebagliati Martins National Hospital in the months November - December 2016, for which information was collected directly from the medical records. of the patients and also of the computerized records of the Unit Dose Pharmacy Service and the Internal Medicine Service - 11C. The clinical histories of the patients with the diagnoses were selected: urinary tract infection, community acquired pneumonia, hospital septicemia and tuberculosis. Once the number of clinical histories of the patients was obtained by pathology, we proceeded to collect the clinical information such as: diagnosis of admission, medications, route of administration, frequency and laboratory tests such as: blood counts, antibiograms, urine cultures and bronchial secretions; all the data obtained were recorded in a pharmacotherapeutic follow-up format provided by the Unitary Unit Pharmacy Service of the HNERM - EsSalud. Clinical effectiveness: determined by means of the normal values of body temperature, heart rate and respiratory frequency recorded in the medical records. The microbiological effectiveness: was determined by means of, the normal values for leukocytes, neutrophils and platelets registered in the clinical histories. The results obtained responded favorably to the pharmacotherapeutic treatment while their study parameters were normal. It is concluded that antibacterial treatment has clinical and microbiological effectiveness since 100% of patients responded favorably to pharmacotherapy
